REP INFORMATION 

Please review the CMHA Rep Letter for the 2010 – 2011 season as it contains very important information pertaining to the Representative divisions.

On-line registration for Returning Players starts May 15th 2010.

Payment with Cheque, Visa or Mastercard. Cheques are preferred and must be received within 2 weeks of registration or registration will be void. 

Cheques are to be payable to CMHA and mailed to:

If you have outstanding fees, they must be paid prior to being allowed to register for this year. 

First time users of online registration must have been registered in CMHA the prior season before they can login. There is a link to register on the right side of the page under "Other Options".

Your Hockey ID number was stated on last years receipt for registration which was recently handed out. It helps to have this number but is not mandatory. If you wish to obtain your Hockey ID #, please contact your Division Coordinator.

Goalies are registered in the system with a space and the letter G in brackets, you must spell your name like that for the system to recognize goalies ie: Smith (G)

2010 NEW PLAYER REGISTRATION MEETING
Saturday, June 12th, 2010

 

LOCATION - MULTI PURPOSE ROOM - NEW ARENA

 

9AM TO 12PM - FOLLOWED BY RETURNING PLAYER REGISTRATION.  This will be the last day to register as returning player with no late fee.  Further, any players not registered by June 30, 2010 will no longer be considered members and will be placed in the order of date of registration on the waitlist. 

New players transferring from another association must have a completed and signed “Player Movement Form,” (Form #133 signed by the old association), as well as a "Declaration of Residence Form," (Form #142) available through www.pcaha.bc.ca at the time of registration. Forms must be signed by both parents even if the player only lives with one. A player movement fee will be assessed and payment is required at time of registration.

New players must provide a photocopy of the player’s birth certificate on a letter size sheet of paper (which will be kept by the registrar).

TWO PIECES OF PROOF OF RESIDENCY in Coquitlam are required for all player registrations: Accepted are property tax notice, BC Hydro, Terasen Gas, Shaw Cable bill or lease/rental agreement. Registration will be rejected unless photocopies of 2 pieces of proof of residency are provided.  Please note that all residency documents will be checked prior to your child's first skate/evaluation and if it is not provided, your child will not be allowed on the ice and your registration could be revoked.

One cheque or money order per child (inc rep tryout fee),per division (except for Juvenile). Cash will not be accepted.

*First year and new to hockey Peewee players must attend a mandatory Body Checking Clinic and will be assessed a  $60.00 fee. Please choose "Peewee Bodychecking Package" in the sign up process.  If you choose to do your bodychecking clinic elsewhere, Inside Edge and RPM are the CMHA approved providers and you must provide your receipt at registration.

There will also be an additional $10.00 assessed per player registration for a Hardship Raffle for which you will be given tickets to sell to recover the $10.00.  We have a signed Canucks Jersey, Vancouver GIants Jersey and Coquitlam Express Jersey for 1st, 2nd & 3rd prizes.

Rep tryout fee for all divisions except Juvenile is $110.00 and in all divisions including Juvenile is NON-REFUNDABLE - NO EXCEPTIONS

 

*All Midget players trying out for Major Midget or Junior camps must provide two cheques for registration - Try-out fee is non-refundable. 

*All Juvenile players must provide two cheques for registration - Try out fee is non- refundable.

Any registered player that obtains a release by September 30th to play for Major Midget or for a Junior team shall receive a 100% refund of the basic registration fees but NOT THE REP TRYOUT FEE

CANCELLATION OF REGISTRATION AND REFUND POLICY

THERE WILL BE NO REFUNDS PROCESSED PRIOR TO OCTOBER 15, 2010

(a) Players or parents choosing to cancel their registration shall notify the Registrar

and appropriate Division Coordinator in writing.

 (b) Refunding of registration fees shall be done in accordance with the schedule set

out by the Executive. The policy refund amounts are as follows:

·         Prior to August 15th 100%

·         August 16th – September 15th 100% less $50 administration fee

·         September 16th – October 1st 75%

·         October 2nd - November 15th 50%

·         November 16th – December 15th 25%

There shall be no refund of fees after December 15th of the current season.

 

(c) Any registered player that obtains a release by September 30th to play for Major

Midget or for a Junior team shall receive a 100% refund of the basic registration

fees.

 


No release will be granted from Coquitlam Minor Hockey to any player with

outstanding financial obligations to the Association and until all equipment on

loan to the player or his/her family is returned.
